The alpha-fodrin antibody is primarily associated with research in autoimmune disorders, particularly Sjögren's syndrome (SS). Alpha-fodrin, a cytoskeletal protein component of the spectrin family, plays a role in maintaining cell membrane integrity and organizing intracellular structures. In the late 1990s, studies identified autoantibodies targeting the 120 kDa alpha-fodrin fragment as potential biomarkers for SS, a chronic autoimmune condition characterized by exocrine gland dysfunction leading to dry eyes and mouth. These antibodies were proposed to correlate with disease activity and glandular damage, offering diagnostic utility alongside traditional markers like anti-SSA/Ro and anti-SSB/La.

However, subsequent research revealed inconsistencies in sensitivity and specificity. While some studies supported alpha-fodrin antibodies as early diagnostic tools, others found limited clinical relevance or overlap with other autoimmune conditions like systemic lupus erythematosus. This variability has led to debates about their diagnostic value. Current investigations focus on clarifying their pathophysiological role, including potential involvement in glandular apoptosis and immune dysregulation. Despite unresolved controversies, alpha-fodrin antibodies remain a subject of interest in understanding SS mechanisms and refining diagnostic criteria.
