Desmoglein 2 (DSG2) is a transmembrane glycoprotein belonging to the desmoglein subfamily of cadherins, primarily involved in forming desmosomes—specialized intercellular junctions critical for tissue integrity and mechanical stability in epithelial and myocardial cells. These adhesion complexes mediate cell-cell interactions through calcium-dependent binding, playing essential roles in tissue morphogenesis, homeostasis, and barrier function. DSG2 is ubiquitously expressed, distinguishing it from other desmogleins (e.g., DSG1/3) with tissue-specific roles.

Antibodies targeting DSG2 have dual contexts: pathological autoantibodies and research/clinical tools. In autoimmune disorders, DSG2 autoantibodies are less commonly implicated than DSG1/3 antibodies (linked to pemphigus). However, emerging studies associate anti-DSG2 antibodies with arrhythmogenic cardiomyopathy (ACM), where they may disrupt desmosomal function, contributing to myocardial inflammation and fibrosis. Such autoimmunity might arise from molecular mimicry post-viral infections or genetic predisposition.

In research, DSG2-specific antibodies are vital for studying desmosome assembly, cell adhesion dynamics, and DSG2’s non-canonical roles in signaling pathways, including EGFR and Wnt. They are also investigated in cancer, as DSG2 overexpression in certain tumors correlates with invasion and metastasis. Therapeutic antibodies or inhibitors targeting DSG2 are being explored to modulate cell adhesion in diseases like cancer or autoimmune conditions. However, their clinical application requires further validation of specificity and safety to avoid off-target effects on normal tissues reliant on DSG2 function.
