Ieramilimab is an investigational, humanized immunoglobulin G4 (IgG4) monoclonal antibody developed for cancer immunotherapy. It functions as a specific inhibitor of the Lymphocyte Activation Gene-3 (LAG-3) immune checkpoint. Its primary mechanism of action is to directly inhibit the interaction between LAG-3 and its major histocompatibility complex class II (MHC-II) ligands. By binding to LAG-3 with high affinity, ieramilimab sterically hinders its engagement with MHC-II molecules expressed on antigen-presenting cells (e.g., dendritic cells) and some tumor cells.
