抗体

抗体是存在于体液和淋巴细胞表面上的能与抗原特异性结合的免疫球蛋白。抗体是由B细胞分化成的浆细胞产生的。抗体分子具有结合部位(结合簇),能与相对应的抗原决定簇结合。由于不同的抗原与抗体特异性结合表现出各种不同的反应,因而抗体有各种不同的名称,如凝集素、沉淀素、抗毒素、溶血素、溶菌素、补体结合抗体等。这些结合,在适当的条件下,可发生直接或间接的可见反应,可作为临床诊断的一种方法。抗体有作用于相对应的细胞、组织、毒素和酶等一系列的生物学活性,与对应抗原在机体内结合后,可以被吞噬、排泄而将抗原清除,或使抗原失去致病作用,故常用以防治某些疾病。这在医学应用中,曾发挥了巨大的作用。在另一些情况下,抗体与抗原结合形成的免疫复合物,能损伤组织、细胞,引起超敏反应或免疫性疾病等不良后果。抗体过去统称为丙种(γ)球蛋白(γG,GG),这是因早年用电泳法分析血清时看到抗体活性在γ部分而命名的。后来证明,抗体并不都在γ区; 反之,位于γ区的球蛋白也不一定都具有抗体活性。1964年世界卫生组织专门会议将具有抗体活性以及与抗体相关的球蛋白统称为免疫球蛋白(Ig)。目前多数学者认为抗体这一名词只表明它的生物学活性,如在靶抗原已被确定的场合。可与靶抗原特异性结合,但不能表明它的化学本质。而免疫球蛋白一词既意味着有免疫活性,更着重表示它的化学含义,是指单一的分子。不严格区分时,二词可以混用。B淋巴细胞受抗原刺激后,怎样变成能够分泌抗体的浆细胞或转变成记忆细胞,这些机制还远未搞清。


抗体的产生有一定的规律。当抗原第一次进入机体时经过一段潜伏期,抗体效价逐步上升,达到高峰后,维持一短暂的时期,以后又逐渐下降,此为初次应答或初次抗体应答。但在第二次再接触同样抗原后,开始时表现为原有抗体量的略为降低。这是因为原有抗体的一部分与再次注入的抗原结合的缘故。随后抗体效价的上升较初次为快,其高峰时的效价也比初次为高,持续的时间也长,此即所谓二次(抗体)应答或再次(抗体) 应答。由抗原刺激所产生的抗体,经过一段时间后即逐渐消失,以后再接触抗原,可使已消失的抗体迅速上升,是为回忆应答。广义的回忆应答可包括二次应答。若再次刺激的抗原与初次相同,则为特异性回忆应答,若与初次不同,则为非特异性回忆应答。非特异性回忆应答的抗体只是暂时性上升,短时间内即很快下降。
抗体的一般特性包括: ①特异性: 抗体只能与相应抗原发生特异性结合,而不能与其他无关的抗原结合。这是由抗体的结合部位与抗原决定簇的理化性质恰相对应所决定的。②不均一性: 抗体是一组不同种类的免疫球蛋白分子所组成(参见“免疫球蛋白”)。它们的氨基酸组成和排列以及立体构型既相似又略有差别,因而其大小、形状和功能也互有差异。例如天然抗原性物质的表面含有许多不同的决定簇,由其刺激机体产生的抗体,就包含着针对这些不同决定簇的抗体。例如抗伤寒杆菌的抗体,至少应包括抗菌体的O抗体,抗鞭毛的H抗体以及抗毒力 (Vi)抗原的Vi抗体; 事实上针对菌体的抗体,由于菌体抗原(9,12)不同,其相应抗体也互不相同。就是针对某一决定簇的抗体,也包括IgG、IgA、IgM、IgD和IgE五大类及其亚类,虽然在某一个体内不一定同时都有。对某一类抗体(如IgG)来说,不同种系以及不同个体动物间所产生的抗体也都不完全相同。这种不同是由遗传决定的。抗体的这些不均一性,在临床上都是必须注意的问题。③双重性: 抗体一方面可与抗原特异性结合,发挥其免疫功能; 另一方面,由于它本身是高分子蛋白质,当被注入异种动物体内时,就具有抗体和抗原的双重作用。在用异种动物的抗血清反复注射以防治疾病时,必须注意有发生过敏反应的可能性。关于抗体产生的机制,曾有过许多假设和学说。


人类免疫球蛋白已鉴定出5种,它们是:IgG,IgA,IgM,IgD和IgE,差异发在H链上,分别由称为γ、α、μ、δ和ε的H链构成。
IgG是人类血清中的主要免疫球蛋白。抗原激发后产生的主要抗体类型,就是这种单体。胎儿虽不产生IgG,但它能迅速通过胎盘,故胎儿和新生儿血清中皆能检出母体的IgG。根据重链的不同,IgG可分为4个亚类:γ1,γ2,γ3,γ4。有些抗原激起的抗体反应,4亚类可说旗鼓相当,但有些抗原则是以其中某一亚类为主。作用于糖类的抗体常为IgG2;抗DNA抗体主要是IgG1和IgG3。在各种亚类中,IgG3的合成速率最低,而分解速率最高,血清半衰期最短。有关IgG的各种生物学和物理特性,均见表1及表2。
IgA有单体也有二聚体。它是分泌物中的主要免疫球蛋白,并以二聚体存在,后者是以多肽连接链(J链)和二硫桥互连的。此外,分泌物(涎液、泪液、肠液中的二聚体,还含有1条称为分泌成分(SC)或分泌片(secretary piece)的多肽链。SC似能保护二聚体IgA,免于胃肠道蛋白水解酶的消化作用。它是由局部浆液型分泌性上皮细胞产生的,位于这些细胞的表膜,这使人想到:SC可能是二聚体IgA的受体。SC片越过或由这些细胞间通过时,不断增加,据认为,分子之能藉胞泌作用(与胞饮作用相对)通过粘膜上皮,即缘于此。因此,分泌性IgA的生成,就可认为是不寻常的,因为它是两种不同类型细胞(即抗体分泌性浆细胞和浆膜上皮细胞)的协作产物。按其α重链的不同,IgA可分为IgA1和IgA2两亚类。后者又可按其遗传标记(genetic markers)的不同,分为A2m(1)和A2m(2)两亚种。
IgM是最大的免疫球蛋白(分子量890,000),为5个相同亚单位组成的五聚体,各有两条K或λ轻链和两条μ重链。此外,IgM还有1个与二聚体IgA相似的J链,使分子结合更为牢靠。IgM对补体结合至为有效,对红细胞和细菌的凝集作用以及杀菌反应,也比IgG有效得多。单体IgM见于B细胞表面,据信是起抗原受体的作用。初次给予1剂抗原后,经过一段起始延搁,首先出现的抗体就是IgM,一般于7日到达顶峰。但此时IgG抗体亦可检出,一般于10~14日到达最高效价。在IgG效价增长期间,IgM效价开始下降,迨至初次抗原剂量4~5周后,已难以检出。2次反应(回忆反应或增强反应)时,IgM和IgG效价皆呈指数性增长,但IgG反应显然更强,持续也更久;IgM效价则与1次反应时相同,或略有增高。产IgM细胞记忆功能不佳,虽经2次抗原激发,亦无典型回忆反应。故IgM的存在有助于确定是否为新发感染。有证据提示带IgG或IgA淋巴细胞是由带IgM或IgM/IgD淋巴细胞前体发育来的。
IgD是由两条δ重链和两条κ或λ轻链构成的单体。在血清免疫球蛋白中,含量甚微,合成速率大约不到IgD的1/100。IgD的分解速率亦高,血浆半衰期只有3日。IgD不与补体结合,也不对皮肤致敏。大多数脐带和成人的B细胞表面,都有IgD,有人认为IgD可能是一种早期抗原细胞受体。
IgE是由两条ε重链和两条κ或λ轻链构成的大单体。血清含量至微,血浆半衰期最短,合成速率最低,分解速率在各种免疫球蛋白中也是最快的。IgE在即发型超敏反应中起主要作用。它以Fc部分紧密地结合在肥大细胞和碱性细胞的受体上。继而抗原(变应原)与临近二IgE分子相互作用,形成抗原桥。由此引起的局部膜改变,促使介质释出(组胺、慢反应物质、酸性细胞趋化因子),从而触发即发型超敏反应。特应性(atopic)者的变应原特异性IgE水平增高。据认为IgE对寄生虫有保护作用,但尚无确切资料得以证实。
