NARF (Neuronal Anti-Ribosomal Factor) antibodies are a subset of autoantibodies implicated in neurological and autoimmune disorders. Initially identified in studies exploring paraneoplastic neurological syndromes, these antibodies target ribosomal proteins or associated factors within neuronal cells, potentially disrupting protein synthesis and cellular function. Their discovery emerged from research linking autoimmune responses to ribosomal components in conditions like systemic lupus erythematosus (SLE) and neuropsychiatric lupus, where ribosomal P proteins are common targets. NARF antibodies, however, are distinct in their specificity for neuronal ribosomal factors, suggesting a unique role in neuroinflammation or neurodegeneration.

While their exact pathogenic mechanism remains unclear, NARF antibodies are hypothesized to interfere with ribosome-mRNA interactions, impairing synaptic plasticity or neuronal repair. Clinically, they are associated with rare autoimmune encephalitides, cerebellar ataxia, or overlap syndromes with psychiatric manifestations. Detection typically involves cell-based assays or immunohistochemistry on brain tissue, though standardized diagnostic protocols are still evolving. Research continues to clarify their diagnostic and prognostic value, as well as their potential role in bridging autoimmune dysregulation and neurological dysfunction.
