Granulysin (GNLY), a cytolytic and proinflammatory protein expressed in human cytotoxic T lymphocytes and natural killer (NK) cells, was first identified in the late 1990s. It belongs to the saposin-like protein family and shares structural homology with antimicrobial peptides. GNLY exists in two isoforms: a 15-kDa precursor and a proteolytically processed 9-kDa active form. It is co-stored in cytotoxic granules with perforin and granzymes, playing dual roles in immune defense.

Functionally, GNLY exhibits broad antimicrobial activity against bacteria, fungi, parasites, and viruses by disrupting microbial membranes. Notably, it contributes to pathogen clearance in intracellular infections like tuberculosis. In cancer immunity, GNLY induces apoptosis in tumor cells through membrane disruption and mitochondrial targeting. Beyond direct cytotoxicity, it acts as a danger-associated molecular pattern (DAMP), modulating immune responses by activating dendritic cells and promoting inflammation.

GNLY antibodies are crucial research tools for studying its expression patterns, trafficking mechanisms, and therapeutic potential. Elevated GNLY levels correlate with favorable outcomes in infections and certain cancers but may exacerbate tissue damage in inflammatory disorders like Stevens-Johnson syndrome. Recent studies explore its role as a biomarker for cytotoxic immune activation and its therapeutic applications in antimicrobial or antitumor strategies.
