Benralizumab is a humanized, afucosylated monoclonal antibody of the IgG1/kappa isotype approved for the add-on maintenance treatment of severe eosinophilic asthma. It specifically targets the interleukin-5 receptor alpha subunit (IL-5Rα). Unlike other anti-IL-5 biologics that neutralize the cytokine itself, benralizumab binds directly to IL-5Rα on eosinophils and basophils. Its afucosylated Fc region is engineered to dramatically enhance affinity for FcγRIIIa receptors on natural killer (NK) cells and macrophages. This results in potent antibody-dependent cell-mediated cytotoxicity (ADCC), leading to rapid and near-complete depletion of eosinophils through apoptosis (programmed cell death).
