Otilimab (also known as GSK‑3196165 or MOR‑103) is a fully human monoclonal antibody that targets granulocyte‑macrophage colony‑stimulating factor (GM‑CSF), a pro‑inflammatory cytokine. By binding to GM‑CSF with high affinity, it neutralizes the cytokine and prevents it from interacting with its receptor (GM‑CSFR) on myeloid cells, thereby inhibiting downstream inflammatory signaling. This blockade aims to reduce the activation and survival of macrophages and other inflammatory cells that drive tissue damage in autoimmune and inflammatory diseases. It has been clinically evaluated for the treatment of rheumatoid arthritis (RA), osteoarthritis, and other inflammatory conditions, though its development in RA has been discontinued after mixed clinical results.
