Polzastobart is a humanized IgG4 kappa monoclonal antibody developed as an immunomodulatory therapeutic agent. Its primary mechanism of action is the specific and high-affinity binding to the Leukocyte Immunoglobulin-Like Receptor B2 (LILRB2), also known as ILT4 or CD85d. This receptor is predominantly expressed on myeloid cells such as monocytes, macrophages, dendritic cells, and neutrophils. By blocking LILRB2, polzastobart aims to disrupt its interaction with classical and non-classical MHC class I molecules (HLA-G being a high-affinity ligand).
