Ipilimumab is a fully human monoclonal IgG1κ antibody approved for the treatment of advanced cancers, most notably metastatic melanoma. It functions as a cytotoxic T-lymphocyte-associated protein 4 (CTLA-4) checkpoint inhibitor. By binding with high affinity to CTLA-4 on the surface of T cells, ipilimumab blocks its interaction with its ligands, B7-1 (CD80) and B7-2 (CD86), on antigen-presenting cells. This blockade prevents CTLA-4 from delivering an inhibitory signal, thereby "releasing the brake" on early T-cell activation and proliferation. This enhances the priming and expansion of tumor-specific T cells, leading to a more robust anti-tumor immune response.
