What is Azelaic Acid?
Azelaic acid is a naturally occurring saturated dicarboxylic acid. It is found in grains like wheat, rye, and barley. While it occurs naturally, the azelaic acid used in medications and cosmetics is produced synthetically to ensure purity and consistency.
Its applications are almost exclusively in dermatology, leveraging its multi-targeted effects on the skin.
Primary Applications and Uses
Azelaic acid's primary and most significant applications are in the treatment of common skin conditions. It is available both as a prescription drug and in over-the-counter cosmetic products.
1. Treatment of Acne Vulgaris
This is one of its most common uses. Azelaic acid is effective for treating mild to moderate inflammatory acne.
Mechanism of Action:
Antimicrobial: It effectively kills Cutibacterium acnes (the bacteria responsible for acne) inside the follicle.
Anti-Keratinizing: It normalizes the shedding of skin cells inside the follicle, preventing them from clumping together and forming comedones (clogged pores that lead to blackheads and whiteheads).
Anti-inflammatory: It reduces the redness and swelling associated with inflammatory acne lesions (papules and pustules).
2. Treatment of Rosacea (Specifically Papulopustular Rosacea)
It is a first-line treatment for the red bumps and pimples (papules and pustules) of rosacea.
Mechanism of Action:
Its potent anti-inflammatory properties are key here, helping to reduce the redness and number of inflammatory lesions.
It also helps to reduce erythema (overall redness) and may improve the uncomfortable burning or stinging sensations.
3. Treatment of Hyperpigmentation (Melasma and Post-Inflammatory Hyperpigmentation)
Azelaic acid is highly effective at lightening dark spots on the skin.
Mechanism of Action:
Tyrosinase Inhibition: It competitively inhibits tyrosinase, the key enzyme required for the production of melanin (skin pigment). This is the same mechanism used by hydroquinone, a classic skin-lightening agent.
Cytotoxic to Abnormal Melanocytes: It selectively targets and inhibits hyperactive melanocytes (the pigment-producing cells) that are causing the dark spots, while leaving normal cells unaffected. This makes it a safer alternative with fewer side effects like permanent skin lightening (ochronosis).
