Promecaine hydrochloride, also known as Alcaine, propoxyphenocaine hydrochloride, is an ester surface anesthetic. Its mechanism of action is to reduce the instantaneous permeability of neurons to sodium, stabilize nerve cell membranes, prevent the generation and conduction of nerve electrical impulses, and thus produce anesthesia.

By interfering with the binding of calcium ions in nerve tissue and phospholipids on the membrane, promecaine hydrochloride prevents the inflow of sodium ions necessary for generating action potentials and blocks the conduction of nerve impulses, causing local anesthesia.
