The C4b-binding protein alpha chain (C4BPA) is a key regulatory component of the complement system, a critical part of innate immunity. C4BPA primarily functions in the classical and lectin complement pathways by binding to C4b, a cleavage product of complement component C4. to inhibit the formation of the C3 convertase (C4b2a). This activity prevents excessive complement activation, protecting host tissues from unintended damage. C4BPA also interacts with anticoagulant protein S, linking complement regulation with coagulation pathways.

C4BPA antibodies are autoantibodies targeting this protein, often detected in autoimmune and inflammatory conditions. In diseases like systemic lupus erythematosus (SLE) or anti-phospholipid syndrome (APS), anti-C4BPA antibodies may disrupt its regulatory functions, leading to uncontrolled complement activation, inflammation, and thrombotic events. Studies suggest these antibodies correlate with disease severity, potentially serving as biomarkers for diagnosis or prognosis. Research also explores their role in rare disorders, such as atypical hemolytic uremic syndrome (aHUS), where complement dysregulation is central.

Understanding C4BPA antibodies provides insights into complement-related pathologies and may guide therapeutic strategies targeting complement inhibitors or immunomodulation. Further studies are needed to clarify their pathogenic mechanisms and clinical utility.
